You are kindly requested to share the following needed documents to bids_lbn@unfpa.org by Tuesday, May 19:

  • Fill, sign & stamp the RFQ ( i.e. page #1)
  • Complete the price list (Annex I)
  • Complete the Questionnaire for Medical Devices (Annex II)
  • Share the legal status &  minimum documentation as per Annex III
  • Share clear photos of the PPE product and packaging (preferably in a format where the dimensions and features can be visually verified).

Important notes:

  1. The payment will be processed upon the satisfactory receipt of goods from the bank of America, and in accordance with the currency specified in RFQ.
  2. The VAT identification #, delivery date to be reflected in RFQ (gage #1).
  3. Samples may be requested later
  4. Offers sent to another email account will  be disqualified.
  5. Partial bids will be accepted
